Upon receipt of a completed Consent Form, a copy of the original medical record will be directed to you — or a like-regulated healthcare provider of your choice.

Note: To help cover cost of copying and mailing or electronic delivery, we may charge a cost-based fee as permitted by Virginia law. We will let you know that amount before we fulfill your request.
