Ché Frost, MHA, BSN, RN

Ché Frost serves as executive vice president of care coordination at Continuous Care, the nurse-led telehealth services company that provides essential, personalized care support to patients between doctor appointments. In this position, she directs patient-centric care coordination services, working with health systems, hospitals and medical facilities, and oversees the development and implementation of patient care standards, processes and protocols across the continuum of care.

With more than 20 years of experience in the healthcare industry, Ché is known for her expertise in clinical operations, program development and consulting. Before joining Continuous Care, she spent nearly a decade at Elevance Health, most recently as director of health and wellness. In this position, she successfully created, launched and scaled a new clinical organization across more than a dozen states with a focus on improving health outcomes for federal employees.

Ché began her career as a registered nurse, delivering high-quality care in hospitals and medical centers in numerous states. She earned a Bachelor of Science in Nursing degree from the University of Missouri and a Master of Health Administration degree from George Washington University.
